As Lion-Man or Mouse-Man, go through the blocks left of the windmill, break the next set of blocks on your left and fall through. The chest is on the right.

As Hawk-Man, go to the Beach and go left one screen. Fly up through the ceiling, the heart is inside the chest.

You'll need the Thunder Ring, and having the Magical Saber helps a lot.
As Lizard-Man, go to the Desert and drop down into the water beneath the ant lions, then go right and smash the blocks with your head. Just go right and don't die. Equipping the Magical Saber will make you unable to break blocks with your head, which will make the dungeon MUCH easier.

As Mouse-Man, break the blocks to the left of the Village's windmill and fall through. Jump through the third hole and go left to reach The Underground. On this first screen, go all the way to the left until you reach the edge of the screen where two blue snakes are. Jump onto the first step and walk left, you'll go through the floor.
Good luck, this dungeon is just pure pain.

Drop down into the water in the first area of The Beach. Head left where there's a small platform of two blocks. Stand on these and press up to enter a hidden door.

In the Beach, go to the second screen, drop into the water and go right. Near the pillars of increasingly size, between the first (2-blocks tall) and second (3-blocks tall) pillars are 2 breakable blocks. They can be seen in with theoriginal graphics, but in the remake, they are obscured by red seaweed in the foreground.

Go to the Desert, turn right and enter the Canyon, then fly right all the way to the end. In the last screen, there is a gap at the top of the blocks blocking the screen. Go through them to enter the dungeon.
